Office Policies

Payment is required for all services at the time they are rendered unless you are in a prepaid plan in which we participate (see below). For those patients, applicable co-payments and deductibles will be collected at the time of the visit. Our office accepts cash, personal checks and Visa/MC/Discover/American Express. Patients are responsible for any/all charges not paid for by their insurance company. For out-of-network or uninsured patients, our office will collect the cost of medical (non-elective) services performed, up to a pre-arranged amount. We will then bill your insurance company, apply their payment and refund you the difference or bill you for any remaining balance.

If you must cancel or reschedule an appointment, please do so at least 24 hours before the scheduled appointment time. Your courtesy to the doctors and their staff, as well as the other patient for whom an appointment could have been used for, is greatly appreciated.

Adam M. Rotunda, M.D. is a participating provider for: 

• Medicare - Part B Only
• HealthCare Partners Medical Group 
• Prospect Medical Group
• No PPO insurances (he is an Out-of-Network Provider)

Kelly M. Bickle, M.D. is a participating provider for:

• Medicare - Part B Only
• The following PPO Insurance:  Blue Shield of CA, HealthNet and UnitedHealth Care
• She is an Out-of-Network Provider for Aetna, Cigna and Blue Cross

Drs. Rotunda and Bickle accept insurance patients, but please check to see if they are Out-of-Network Providers for your plan. We will bill the insurance company for our services. Please contact your insurance company to learn what your benefits will be for Out-of- Network Providers.

Please note - if you are a patient of Dr. Rotunda -  patients with Medicare Part B insurance must contact their secondary insurance company to see what their benefits will be for Out-of-Network Providers. Medicare patients need to be aware of their secondary coverage. If your SECONDARY insurance coverage IS NOT a supplemental insurance, you may be responsible for a portion of the 20% after Medicare pays their part (80% of allowed amount).
